To Register for Football / Cheerleader or Dance you must follow these
instructions:

  1. Obtain 2 copies of child's birth certificate. (Not needed for returners)
  2. Collect 2 recent photos of the child. (Need Recent Photos from everyone or
    bring child to registration and we will take his picture there)
  3. Fill out and initial the DSYJ Youth Football Registration form in its entirety.
    (Make sure to read all additional obligations and Rules contained therein)
  4. Provide proof of insurance coverage. (Bring us a copy showing current dates)
  5. REGULAR Registration Fee. ($125 Football $100 Cheerleading)+($30 Deposit)
